the terms box (50% down payment, and Balance on Completion) 2. Fill in the down payment information; check number, amount and date, or credit card and expiration date. 3. Review the "Installation Checklist" green form (if provided) fill in the necessary information and sign at the bottom. 4. Please keep copies of both of these forms for your records, and send the originals back to your estimator. You must submit your signed paperwork with the down payment in order to be scheduled.
